Job Community Nurse

We are seeking a dedicated and compassionate community nurse to join our dynamic healthcare team. Key Responsibilities

• Conduct comprehensive health assessments and create individualized care plans for patients in the community.
• Administer medications, wound care, and other therapeutic interventions as per healthcare protocols.
• Collaborate with a multidisciplinary team to coordinate care and ensure continuity of services.
• Provide education and counseling to patients and families regarding disease management, prevention, and health promotion.
• Document all nursing procedures and patient interactions in accordance with regulatory standards.
• Monitor and evaluate patient progress and adapt care plans as necessary.
• Support patients and families through transitions of care and facilitate referrals to community resources.
• Adhere to all safety and infection control policies and procedures.

Qualifications

• Current Registered Nurse (RN) licensure in the state.
• Bachelor of Science in Nursing (BSN) is preferred.
• Minimum of 2 years of nursing experience, with a focus on community health nursing.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills to engage effectively with patients, families, and healthcare teams.
• Ability to work independently and manage a diverse caseload.
• Familiarity with electronic health records (EHR) and telehealth technologies.
• Valid driver’s license and reliable transportation for home visits.
